PHARMACHOLOGIC EFFECT
Antibiotic of a wide spectrum of action. The mechanism of action is associated with the suppression of the first stage of peptidoglycan synthesis of the bacterial cell wall. Being a structural analogue of pyruvate phosphoenol, it competes with the enzyme N-acetyl-glucosamine-3-o-enolpyruvyl transferase, which results in a specific, selective and irreversible inhibition of this enzyme.
It is active against most Gram-negative bacteria: Pseudomonas aeruginosa, Escherichia coli, Proteus mirabilis, Klebsiella pneumoniae; Gram-positive bacteria: Staphylococcus spp. (including Staphylococcus aureus, Staphylococcus epidermidis), Streptococcus spp. (including Enterococcus faecalis).
PHARMACOKINETICS
Absorption is 60%; binding to plasma proteins - 10%. C max when administered orally at a dose of 50 mg / kg - 32 Ојg / ml; with iv administration in a dose of 20 mg / kg - 130 mcg / kg; T 1/2 - 2.2 h. Rapidly penetrates and well distributed in tissues. 90% is excreted by the kidneys with the creation of high concentrations in the urine.
INDICATIONS
Treatment of infectious-inflammatory diseases (caused by microorganisms sensitive to the preparation) of the lower parts of the urinary tract, incl. acute bacterial cystitis, exacerbation of chronic recurrent bacterial cystitis, acute bacterial urovezical syndrome, bacterial nonspecific urethritis, asymptomatic massive bacteriuria (in pregnancy), postoperative urinary tract infection. Prevention of urinary tract infection during surgical interventions and transurethral diagnostic examinations.
DOSING MODE
Is taken internally. A single dose for adults - 3 g, for children - 2 g. The treatment regimen is set individually, taking into account the stage of the inflammatory process, the reactivity of the patient's organism, the sensitivity of microorganisms to phosphomycin.
