PHARMACHOLOGIC EFFECT
Opioid analgesic, a partial agonist of mu receptors with some antagonistic activity against kappa receptors.
The effectiveness is somewhat inferior to morphine. The duration of action is longer than for morphine - about 6 hours, which is apparently due to stronger binding to receptors. Oppresses the respiratory center to a lesser extent than morphine.
PHARMACOKINETICS
After oral administration, it undergoes the "first pass" effect through the liver.

After the / m administration of C max in plasma is achieved for a short time.

After sublingual use, it is absorbed buccally, C max is reached after 90 minutes.

Binding to plasma proteins is 96%.
It penetrates the GEB and the placental barrier. Buprenorphine is metabolized in the liver. T 1/2 in the initial phase is 1.2-7.2 h, in the final phase - 20-25 h. It is excreted through the intestine mainly unchanged, kidneys - in the form of metabolites.
INDICATIONS
Moderate and severe pain syndrome of different genesis.

DOSING MODE
In / m or / in slowly 300 mcg with an interval of 6-8 h. Sublingually - 200 mcg with an interval of 6-8 h.

SIDE EFFECT
On the part of the digestive system: nausea, vomiting, dry mouth.

From the side of the central nervous system: headache, depression, inhibition, oppression of the respiratory center, slowing the speed of mental and motor reactions.

From the cardiovascular system: a decrease in blood pressure.

Other: dyspnea, increased sweating, skin rash.

CONTRAINDICATIONS
Inhibition of the respiratory center, pregnancy, lactation, children under 12 years of age, hypersensitivity to buprenorphine.

PREGNANCY AND LACTATION
Contraindicated in pregnancy, during lactation.

APPLICATION FOR FUNCTIONS OF THE LIVER
Use with caution in renal failure.

APPLICATION FOR VIOLATIONS OF THE FUNCTION OF KIDNEYS
Use with caution in liver failure.

APPLICATION FOR CHILDREN
Contraindicated in children under 12 years.

APPLICATION IN ELDERLY PATIENTS

SPECIAL INSTRUCTIONS

DRUG INTERACTION
Buprenorphine potentiates the effects of opioid analgesics, phenothiazines, anxiolytics, sedatives, hypnotics, anesthetics, ethanol, MAO inhibitors.
