PHARMACHOLOGIC EFFECT
Monocyclic beta-lactam antibiotic.
The nucleus of the molecule is alpha-methyl-3-amino-mono-bactamic acid. Suppresses the synthesis of the cell wall, acts bactericidal, resistant to beta-lactamases of gram-negative bacteria. It is active against gram-negative aerobic pathogens (Salmonella spp., Shigella spp., Proteus spp., Escherichia coli, Klebsiella spp., Neisseria gonorrhoeae, Neisseria meningitidis, Haemophilus influenzae, Pseudomonas aeruginosa, Entero-Citrobacter spp.), Does not act on Gram-positive microorganisms and anaerobes. Possible appointment with allergies to penicillins, cephalosporins and carbapenems. Between the aztreonam and aminoglycosides, there is cross-resistance. Rarely causes dysbacteriosis.
PHARMACOKINETICS
With the / m introduction, absorption is fast.
T max - 1 h. C max - after intravenous administration 0.5, 1 and 2 g - 54, 90 and 204 Ојg / ml, respectively. Passes histohematological barriers and penetrates well into tissues. T 1/2 - 1.7 hours (with normal kidney function), if the excretory function is impaired and in old age - lengthens. It penetrates through the BBB during inflammation of the meninges, penetrates the placenta, is excreted in breast milk. It is slightly metabolized in the liver.60-75% is excreted by the kidneys unchanged, 12% - with bile. With hemodialysis, the concentration in the blood decreases by 30-60%.
INDICATIONS
- sepsis;

- endocarditis;

- meningitis;

- peritonitis;

- infections of the lower respiratory tract (pneumonia, lung abscess, pleural empyema, bronchitis);

- Gastrointestinal tract infections;

- bile duct infection (cholangitis, cholecystitis, enterocolitis);

- infections of the kidneys and urinary tract, urogenital area (pyelonephritis, urethritis, cystitis, endometritis);

- Gonorrhea;

- infections of bones, skin and soft tissues;

- localized parenchymatous abscesses;

- infected burns and wounds;

- infection or threat of infection in patients with reduced immunity;

- Postoperative infections (including prevention).

DOSING MODE
In / m, in / in streamwise and drip.

When pyelonephritis and infections of the urinary tract are prescribed in / m 0.5-1 g 2-3 times a day.

For systemic infections of moderate -to- moderate course , 1-2 g 2-3 times a day;
with systemic infections of severe course - 2 g 2-4 times a day. The maximum daily dose is 8 g.
In the treatment of acute uncomplicated gonorrhea and acute uncomplicated cystitis - once per / m 1 g.

With sepsis, peritonitis is injected / in;
for jet intravenous administration the preparation is dissolved in 6-10 ml of water for injection; injected for 3-5 minutes; for intravenous drip infusion 1 g of the drug is dissolved in 3 ml of water for injection and diluted in 50-100 ml of 0.9% sodium chloride solution.
Children over the age of 9 months are prescribed 30 mg / kg 3-4 times / day;
children older than 2 years with severe infections - 50 mg / kg 3-4 times / day.
In chronic renal failure (KK 10-30 ml / min), appoint half the average single dose.

SIDE EFFECT
Allergic reactions: skin rash, skin itching, multi-form exudative erythema, petechia, urticaria.

From the digestive system: diarrhea, nausea, vomiting, stomatitis, taste change, candidamycosis.

From the nervous system: convulsions, diplopia, insomnia, dizziness, headaches, confusion.

From the musculoskeletal system: myalgia, arthralgia.

Laboratory indicators: eosinophilia, thrombocytopenia, increased prothrombin time, anemia, neutropenia, increased activity of "liver" transaminases (ALT and AST) and APF.

Local reactions : with iv administration - phlebitis, thrombophlebitis, unpleasant sensations at the site of the / m administration.

Other: swelling of the mammary glands, increased sweating.

CONTRAINDICATIONS
hypersensitivity;

- chronic renal failure (CC less than 10 ml / min);

- liver failure;

- Pregnancy;

- lactation period;

- Children's age (up to 9 months).

C with caution: allergic reactions to other beta-lactam antibiotics in the anamnesis.

PREGNANCY AND LACTATION
Contraindicated in pregnancy and lactation.

APPLICATION FOR FUNCTIONS OF THE LIVER
Contraindicated in chronic renal failure (CC less than 10 ml / min).

APPLICATION FOR VIOLATIONS OF THE FUNCTION OF KIDNEYS

OVERDOSE

Pharmaceutically incompatible with heparin and metronidazole.
It has a synergistic effect with vancomycin, clindamycin, aminoglycosides, penicillins, cephalosporins, carbapenems, metronidazole.
