PHARMACHOLOGIC EFFECT

Combined drug regulating the exchange of calcium and phosphorus in the body.
Reduces resorption and increases bone density, making up for the lack of calcium and colcalciferol (vitamin D 3 ) in the body.
Колекальциферол (vitamin D 3 ) improves absorption of calcium from the digestive tract and its binding in bone tissue.

The use of the drug prevents the increase in the synthesis of parathyroid hormone, which is a stimulator of increased bone resorption.

PHARMACOKINETICS

Kolekaltsiferol is absorbed from the small intestine, and calcium in ionized form from the proximal part of the small intestine through an active vitamin-D-dependent transport mechanism.

INDICATIONS

As a therapeutic and prophylactic agent for:

- prevention and complex therapy of osteoporosis (including menopause, senile, steroid, idiopathic) and its complications (bone fractures);

- Renewal of calcium and vitamin D deficiency in elderly patients.

DOSING MODE

Before using the drug should consult a doctor.

The tablet is chewed or absorbed (should not be swallowed whole).

As a preventive measure

Adults appoint 1-2 tablets / day, mainly during meals.

SIDE EFFECT

CONTRAINDICATIONS

- hypercalcemia (including those with hyperfunction of parathyroid glands);

- hypercalciuria;

- urolithiasis (formation of calcareous stones);

- sarcoidosis;

- metastasis of tumors in the bone;

- Osteoporosis caused by prolonged immobilization;

- an overdose of vitamin D 3 ;

- pulmonary tuberculosis in active form;

- phenylketonuria;

- intolerance of some types of sugars (including lactose, sucrose, sorbitol, fructose);

- Hypersensitivity to the components of the drug (including peanuts, soybeans).

Caution should be applied to the drug in renal failure, benign granulomatosis.

PREGNANCY AND LACTATION

In pregnancy and during lactation, the daily dose of the drug should not exceed 1500 mg of calcium and 600 IU of colcalciferol (not more than 1 tab. / Day).

Overdose of the drug during pregnancy can lead to disruption of the child's mental and physical development.

Calcium and colcalciferol are excreted in breast milk.

APPLICATION FOR FUNCTIONS OF THE LIVER

The drug is contraindicated in severe violations of kidney function, hypercalciuria, urolithiasis (formation of calcareous stones).

Caution should be used in cases of kidney failure.

SPECIAL INSTRUCTIONS

During the treatment with the drug should always monitor the excretion of calcium in the urine and the concentration of calcium and creatinine in the blood plasma.The dose should be reduced or temporarily discontinued if the concentration of calcium or serum creatinine is increased, the level of calcium in the urine is greater than 7.8 mmol / 24 h (300 mg / 24 h).

When using the drug should take into account the additional intake of vitamin D 3 from other sources.
It is not possible to take simultaneously with multivitamin complexes containing calcium and vitamins of group D.
To prevent the absorption of bisphosphonate or sodium fluoride, it is recommended to take Natekal D 3 not earlier than 3 hours after their application.

GCS decreases the absorption of calcium, so the hormone therapy may require an increase in the dose of the drug Natekal D 3 .

In elderly patients, the need for calcium is 1.5 g / day, vitamin D 3 - 1000 IU / day.

OVERDOSE

Symptoms: thirst, polyuria, decreased appetite, nausea, vomiting, constipation, dizziness, weakness, headache, fainting, coma;
hypercalciuria, hypercalcemia (calcium in plasma about 2.6 mmol). With prolonged use of the drug - calcification of blood vessels and tissues.
Treatment: rehydration, "loop" diuretics, GCS, calcitonin, bisphosphonates, diet with calcium restriction, hemodialysis.

DRUG INTERACTION

With the simultaneous use of the drug Natekal D 3 with phenytoin or barbiturates, the activity of colcalciferol decreases.

With simultaneous use with cardiac glycosides, there is an increase in their effect due to calcium, which is part of the drug Natekal D 3 .
Therefore, with this combination, it is necessary to monitor the ECG and the clinical state of the patient.
With simultaneous use with antibiotics of the group of tetracyclines, calcium and colcalciferol increase the absorption of tetracyclines from the gastrointestinal tract.Therefore, the interval between taking tetracyclines and Natekal D 3 should be at least 3 hours.

With simultaneous application of the drug Natekal D 3 reduces the absorption of bisphosphonates and sodium fluoride.

GCS decreases calcium absorption.

With the simultaneous use of diuretics from the group of thiazides, the risk of hypercalcemia increases due to an increase in the reverse absorption of calcium from the lumen of the renal tubules.
Furosemide and other "loop" diuretics increase the excretion of calcium by the kidneys.
When used simultaneously with colestyramine and laxatives based on vegetable or mineral oil, the absorption of colcalciferol decreases.

The acid reaction of foodstuffs increases, and the slightly alkaline reaction reduces the absorption of calcium.
