INSTRUCTION FOR THE SPECIALIST.

Description of the drug approved by the manufacturer for the printed edition of 2011.

PHARMACHOLOGIC EFFECT

Participates in various metabolic processes in the body, enhances oxidation-reduction processes in the body, improves the antitoxic function of the liver.
The infusion of dextrose solutions partially replenishes the water deficiency. Dextrose, entering the tissues, is phosphorylated, turning into glucose-6-phosphate, which is actively included in many parts of the body's metabolism. It is absorbed completely by the body, the kidneys are not excreted (the appearance in the urine is a pathological sign).
Isotonic 5% dextrose solution has detoxification, metabolic action, it is a source of valuable easily digestible nutrient.
With the metabolism of glucose in tissues, a significant amount of energy necessary for the life of the organism is released.
Hypertensive 10% glucose solution increases osmotic blood pressure, improves metabolism;
increases myocardial contractility; improves the antitoxic function of the liver, dilates blood vessels, increases diuresis.
INDICATIONS

Hypoglycemia, insufficiency of carbohydrate nutrition, toxicoinfection, intoxications in liver diseases (hepatitis, dystrophy and atrophy of the liver, including hepatic insufficiency), hemorrhagic diathesis;
dehydration (vomiting, diarrhea, postoperative period); intoxication; collapse, shock. It is used as a component of various blood-substituting and anti-shock fluids.
DOSING MODE

In / in a dropwise 5% solution is injected at a maximum rate of up to 7 ml (150 cap / min) (400 ml / h);
the maximum daily intake for adults is 2000 ml. 10% solution is injected at a maximum rate of up to 3 ml (60 cap) / min; the maximum daily dose for adults is 1000 ml.
In / in the jet - 10-50 ml of 10% solution.

When used for parenteral nutrition in adults with normal metabolism, the daily dose of dextrose administered should not exceed 4-6 g / kg / day, i.e.
about 250-450 g / day (with a decrease in the metabolic rate, the daily dose is reduced to 200-300 g), while the volume of the injected liquid is 30-40 ml / kg / day. The rate of administration: at the normal state of metabolism, the maximum rate of administration to adults is 0.25-0.5 g / kg / h (with a decrease in metabolic rate, the rate of administration is reduced to 0.125-0.25 g / kg / h).
Children for parenteral nutrition, along with fats and amino acids, on the first day, 6 grams of dextrose / kg / day, followed by up to 15 g / kg / day.
When calculating the dose of dextrose when 5% and 10% solutions are administered, the allowable volume of the injected fluid must be taken into account; for children with a weight of 2-10 kg - 100-165 ml / kg / day, children with a weight of 10-40 kg - 45-100 ml / kg / day. The rate of administration should not exceed 0.75 g / kg / h.
Patients with diabetes mellitus are administered dextrose under the control of its content in the blood and urine.

SIDE EFFECT

Violation of the water-electrolyte balance, hyperglycemia, fever, hypervolemia, acute left ventricular failure.

CONTRAINDICATIONS

Hyperglycemia, diabetes mellitus, hypersensitivity, hyperlactacidemia, hyperhydration, postoperative disorders of glucose utilization;
edema of the brain, pulmonary edema, acute left ventricular failure, hyperosmolar coma.
Carefully.
Decompensated heart failure, chronic renal failure (oligo-, anuria), hyponatremia, diabetes mellitus.
PREGNANCY AND LACTATION

5% and 10% dextrose solutions for infusions can be used according to indications during pregnancy and during lactation

APPLICATION FOR FUNCTIONS OF THE LIVER

With caution: chronic renal failure (oligo-, anuria).

APPLICATION FOR CHILDREN

The application is possible according to the dosing regimen.

SPECIAL INSTRUCTIONS

For more complete assimilation of dextrose, administered in large doses, simultaneously with her, insulin is prescribed at the rate of 1 unit of insulin per 4-5 g of dextrose.

Influence on the performance of potentially hazardous activities requiring special attention and rapid reaction.
There is no data indicating a negative effect of dextrose solution on the ability to drive vehicles and engage in other potentially hazardous activities that require increased concentration and speed of psychomotor reactions.
OVERDOSE

Symptoms: hyperglycemia, glucosuria, disturbance of water-electrolyte balance.

Treatment: stop the injection of glucose solution, introduce insulin;
symptomatic therapy.
DRUG INTERACTION

When combined with other drugs, it is necessary to visually control the pharmaceutical compatibility.
